Thanks to Anne Collison Johnson, a Note about Sandra Butler Smith - Sas died of an asthma attack in 2002.  I  had dinner with her a short time before - she was fine then, recently retired and looking forward to new projects.  Her death was a shock to everyone.  Sas was well known as a very respected California Judge (San Joaquin County) and advocate for abused children.  Her books on child abuse are still  available.  A building in Stockton is named in her honor.  Her death  was front page news in Stockton and Sacramento papers.  Sas had three daughters and was divorced.
